Transitions

Serves Brown and Sheboygan County

Transitions is an intervention program that assists youth at Lincoln Hills School in transitioning back to Brown or Sheboygan Counties. Services include individual and group sessions once per week during incarceration at Lincoln Hills on a variety of topics, with an emphasis on employability and life skills. Program staff continue to provide youth with assistance through their release back to the community. Youth in the program receive services for 1 - 2 years after release from Lincoln Hills School.

Service plans are individualized according to youth and family needs and may include:

     -   Advocacy, homework assistance

     -   Employability skills development, job search

     -   Career exploration and planning

     -   Leadership development

     -   Life Skills Education

     -   Rent, Utility, Basic Needs Assistance

Admission Criteria:

Youth ages 14 - 21 incarcerated at Lincoln Hills School for a minimum of 12 months and scheduled for release to Brown or Sheboygan Counties are eligible to participate.

Funding Source:

Transitions is funded by the Bay Area Workforce Development Board. No fee is required to participate.
